Effective July 1, 2019, there is a $25.00 fee to transfer your ACTIVE CNA registration from another state to the Illinois Department of Public Health, Health Care Worker Registry. Please verify your status on the Registry from your state prior to submitting an application to transfer to Illinois.
In order to meet the requirements for transfer, you must have been deemed competent on another state’s registry for CNAs
- Have documentation of current registration from another state indicating requirements listed in the Code of Federal Regulations (42 CFR, Sections 483.151 and 483.152) have been met.
- Must have no administrative findings of abuse, neglect or misappropriations of property on home state’s CNA registry or any other state’s CNA registry.
- Must have no disqualifying convictions on home state’s registry.
To become a CNA via OUT-OF-STATE APPLICATION, download the following pdf form [ Out of State Application ] and mail the completed application with the required $25.00 processing fee to Southern Illinois University Carbondale, Nurse Assistant/Aide Testing, 1840 Innovation Drive, Suite 103, Carbondale, IL 62903.
PLEASE NOTE THAT THERE IS A FEE OF $25.00 WHICH MUST BE SENT WITH THE OUT OF STATE APPLICATION. THE FEE MUST BE MADE PAYABLE TO SIUC BY MONEY ORDER OR CASHIER’S CHECK. NO PERSONAL CHECKS OR CASH WILL BE ACCEPTED.
The IDPH Health Care Worker Registry is aware of this fee and is taking steps to update their website with the information. Absence of the fee language on the Registry website does not override the requirement of the fee in order to gain reciprocity.
If a CNA certification is not active in any other state, then the CNA may recertify in Illinois and no transfer fee is required. Please contact the Nurse Aide Testing office at (877) 262-9259 or by email at email@example.com to inquire as to the process of recertification.
